 <li>Choose a lookup from the drop down to remove.</li>
 <li> A prompt is displayed up to confirm the delete.  Click 'Delete' to
confirm.</li>
 <li>Otherwise click cancel to return to the "Edit Document Lookups'
main page.</li>
